Palliative Care Social Worker

Hours: 22.5 hours per week (days to be negotiated/discussed)

What is the role?

As a Social Worker, you will manage a caseload, working directly with patients, families, and carers to provide coordinated family and bereavement support services of the highest standard. The role involves delivering holistic care that addresses emotional, practical, psychosocial, cultural, and spiritual needs, both in the hospice and in patients’ homes. You will play a pivotal part in empowering patients and their families to navigate life-limiting illnesses, offering advocacy, practical planning, and children’s advice while ensuring equitable access to care and well-being. Pre- and post-bereavement care will also be a central component of your work, supporting families through these challenging transitions.

Collaboration is key in this multidisciplinary role, requiring strong relationships with the care team, statutory services, and partner agencies to ensure seamless transitions for patients and their families. You will also mentor and support staff, volunteers, and students in social care practices, contributing to service development through training, research, and quality improvement. By safeguarding patient welfare and enhancing the expertise of the team, you will help maintain the organization’s commitment to providing expert care and unwavering support, enabling patients and their loved ones to live as fully and comfortably as possible.

About you:

  • Recognised Social Work Qualification or equivalent (eg DipSW, CQSQ, CSS).
  • Palliative care experience.
  • Registration with the Health and Care Professions Council.
  • A minimum of three years’ experience in adult social work, or relevant experience of working with adults and children at risk in similar service areas.
  • Excellent interpersonal and communication skills to build relationships and engage successfully with staff, volunteers, patients, their families and carers.
  • Knowledge of relevant legislation, regulations, codes of practice, processes and procedures.
  • Full UK driving licence and the access to a vehicle to be able to travel across throughout our community in Swindon, North Wiltshire, and into Oxfordshire and Gloucestershire.

 It would also be great if you have:

  • A keen interest to develop this area of expertise.
  • Palliative care qualification (desirable).
  • Relevant experience of working with children at risk or in child protection.
  • Training qualification or experience in delivering training.
  • Mentoring skills.
