Prospect@Home Nurse Coordinator

Are you a registered nurse looking to use your expert clinical skills in a caring, supportive and encouraging environment? Do you have some experience in or a strong desire to work in palliative care? If so, we have an exciting opportunity for you to join our Prospect@Home team as a Nurse Coordinator.

Our focus is on providing outstanding, personalised care for individuals at the end of their lives, living in their own homes in Swindon and North East Wiltshire. You will be responsible for the efficient and effective coordination of responsive, individualised care for patients and to those who are important to them.

Hours: Part time 28 hours per week. Flexible, 3 x 7.5 hours and 1 x 5.5 hours (to include weekends/bank holidays)

What is the role?

The Prospect@Home Coordinator is responsible for the coordination of a high quality, flexible, rapid response and integrated nursing service that will enable patients with life limiting illnesses to be cared for at home, and to die at home if that is their preference.

The prime purpose is to coordinate the delivery of the 24 hour Prospect@Home service. In conjunction with the multi-professional team, a key element will be to assess, monitor and review ongoing patient and family care needs.

Another crucial aspect will be to triage incoming calls to the hospice from patients, families and health and social care professionals and ensure appropriate management and onward referral. This role includes visiting patients in their own home, providing care including assessment and planning of ongoing care.
